CONNECTED EAST KILBRIDE – SELF-MANAGEMENT PROGRAMMES

EK COMMUNITY CONNECTORS

This service is delivered in partnership with Kilbryde Hospice, providing support in the East Kilbride locality, for people who are over the age of 50, experiencing mental health problems or frailty. Our team support people within their communities by identifying what matters to them and then providing support to connect with local activities, pastimes, clubs and services.

We work with people for however long it takes them to feel able to connect with local activities and pastimes, and then providing ongoing support as needed.

CONNECTED EAST KILBRIDE

Connected East Kilbride, a formal collaboration between Healthy and Active in East Kilbride, Kilbryde Hospice and East Kilbride Citizens Advice Bureau, was established in 2021 with the aim of working together to connect local people with services and supports, and helping to improve physical and mental health and wellbeing, reducing social isolation and loneliness, addressing inequalities, and supporting people to live well within their local communities.

Investment has been attracted since 2021 which has helped to drive forward local ambitions by working collaboratively with a diverse range of local organisations and public partners, who share our vision of connecting and supporting people to lead healthy and active lives.

CONNECTED CALDERWOOD

Significant progress has been made since 2021, including the success of Connected Calderwood, which included working with local organisations, mapping the area, identifying local challenges and gaps in provision, and worked collaboratively to address these wherever possible.

Work is ongoing to help people access support within their neighbourhood, whether that is meeting friends, mental health peer support, exercise classes or outdoor activities. There’s lots going on in Calderwood!

This work also highlighted that each neighbourhood in East Kilbride has similar challenges but also unique challenges which need to be addressed locally. The success of Connected Calderwood meant that we were then able to work in partnership in two other neighbourhoods Greenhills and The Murray.

CONNECTED GREENHILLS AND THE MURRAY

CommunityUnity aims to support local people and organisations by identifying resources within these communities, assessing gaps in provision, and working with local organisations and communities to come up with solutions.

This may be a mental health support group, access to information and advice, peer support or more localised challenges including rising levels of poverty and a lack of community spaces where people can meet.

There’s lots happening in these neighbourhoods and plans in the pipeline!

EAST KILBRIDE HEALTH AND SOCIAL CARE FORUM

East Kilbride Health and Social Care Forum meets every six weeks and is an opportunity for individuals and representatives from groups/organisations that have links to the wider community to come together and having a voice in Health and Social Care.

The aim of the East Kilbride Health and Social Care Forum is to:

“Empower patients, service users, carers, and the general public to have a voice in the planning and development of local health and social care services, including those delivered jointly with third sector organisations”.

Currently there are four active Forums in East Kilbride, Hamilton, Clydesdale, and Cambuslang/ Rutherglen that meet regularly and support the wider public to be engaged in discussions and decisions on how to improve health and social care services

CONNECTED EAST KILBRIDE – SELF-MANAGEMENT PROGRAMMES

Delivered in partnership with Kilbryde Hospice, the self-management programme supports people who are waiting for a specialist health or social care intervention or rehabilitation support, particularly those living with long-term conditions, disabilities, or cancer, and enabling them to develop self-management capabilities while awaiting treatment.

The programme offers access to initial weekly sessions covering topics including long-term condition management, pain management, nutrition, diet and physical activity. Participants can then select from a menu of ongoing self-management support through exercises classes, wellbeing and peer support and activities which are available on a flexible basis.
